Below are links to all required forms for Mid-City employees to complete. If you have already submitted these, you do not need to do anything additional.
Site Entry Form: This form asks health questions to assess potential exposure to Coronavirus and must be completed prior to entry to Mid-City’s office or job sites.
Travel Form: We are requesting that all employees disclose any travel within the past 30 days, and any travel they are planning for in the next six months, using this form. Mid-City’s travel policy is aligned with the CDC and State of Ohio.
Update Contact Form: We want to have your most current contact information on hand should we need to update you. It is very important that we have your most current mobile number and email address (if you do not have a company email.)
