Work Site Entry Forms & Daily Temperature Readings

We are requesting that all employees do the following:

  1. Complete a Daily Site Entry Form.
    Office visits: Mobile forms are available here.  Staff, vendors, and customers-please complete prior to entering the building. The form is mobile friendly.

    Field:
    Hard copy forms will be made available on all job sites. At these sites, foremen will pass out and collect these forms from their crew at the start of their shift. All foremen must review these forms and file in a designated folder. (More specific instructions to foremen regarding the review process will be sent to foremen in an individual email.) We have also placed a link to this  printable form here for your convenience. We do plan to make this system electronic, and will have more information to foremen and the service and small projects team early to middle of next week. For the time being, if you do not have a printer on-site, contact Monica at mpoirier@midcityelectric.com.
  2. Take your temperature daily before coming to work. If you are running a temperature of 100.4 degrees Fahrenheit or higher, stay home. Notify your supervisor immediately. If you do not have a thermometer at home, please purchase one, as this is something that we’re all being asked to do regardless of our work commitments. We have thermometers available on some sites, and have ordered more. However, due to the virus, deliveries are more delayed than usual. Please understand its all of our responsibility to self-monitor, and temperature taking is an important part of that.
  3. Do your part. 
    a. Educate yourself and stay on top of updates put out by the CDC.
    b. Wash your hands.
    c. Report anyone you are working with that appears sick.
    d. Stay home when sick.
    e. Avoid close contact.
    f. Cover coughs and sneezes.

Also, most of our office staff is working remotely at this point to limit close contact. Those who are in the office will be at Eastwind Drive for limited hours throughout the day. If you need to come to the office for any reason, contact your supervisor prior for approval.
